Prior to registering as a new patient with us, please follow steps below:
- Have your GP sign the Patient Referral Form.

Have your GP complete and sign the Health Summary Consent Form covering your:
- Current medications & treatments.
- Past medical history including medications used previously.
- Known allergies.
- Ensure that you have signed the Medicinal Cannabis Consent Form.
Not sure how to ask for a referral from your normal GP? Print out this letter and take it with you to give to your GP when you are ready to seek a referral from them.
Once you have completed all of the above, proceed below to register as a new patient, where you will be asked to upload your documents.
